Argatroban Drip for use in HIT

Dosing Protocol            
Mix 250mg in 250ml to make 1mg/ml             
Treatment of HIT: Start with 2mcg/kg/min   
Obtain a baseline aPTT level,then Monitor aPTT levels until the   
aPTT is 1.5-3 times baseline value (obtain level every 2 hours)  (Do not exceed aPTT of 100 seconds)  
Patients with moderate hepatic impairment initiate with 0.5mcg/kg/min
